Consumer Health Data Notice

This Consumer Health Data Notice (“Notice”) supplements our Privacy Policy and applies to consumer health data as defined by the Washington My Health My Data Act (“MHMDA”) and any similar state laws. If there is a conflict between this Notice and our Privacy Policy regarding consumer health data, this Notice controls.
1. Scope
This Notice applies to Washington state residents and other consumers protected by applicable consumer health data laws. It describes how Tea and Slippers (“Company,” “we,” “us,” or “our”) collects, uses, shares, and protects consumer health data when you use our website, forms, tools, and related services.
2. Consumer Health Data We Collect
We may collect the following categories of consumer health data when you voluntarily provide them through our forms, quizzes, calls, chats, or other interactions:
- care needs and health-related circumstances, such as mobility needs, diagnoses, medications, and memory-related conditions;
- activities of daily living (ADLs) and functional limitations;
- veteran status when relevant to care placement or benefits;
- care type preferences, such as assisted living, memory care, or skilled nursing; and
- any other health-related information you choose to provide in connection with your inquiry.
We ask that you provide only the information reasonably necessary for your request. We do not collect consumer health data from sources other than directly from you without your consent.
3. Purposes of Collection and Use
We collect and use consumer health data to:
- respond to your inquiry and match you with relevant senior living providers, communities, or care partners;
- communicate with you about your care search, tours, follow-up, and related services;
- operate, improve, and secure our website, forms, and services; and
- comply with legal obligations.
We do not use consumer health data to build advertising audiences, serve targeted ads, or send health-related information to advertising platforms.
4. Third Parties We Share Data With
We may share consumer health data with the following categories of third parties, solely for the purposes described in this Notice:
- Senior living providers and care partners — communities, agencies, and partners relevant to your inquiry, so they can evaluate fit and respond to you;
- Service providers — vendors that support our CRM, communications, call tracking, appointment scheduling, and business operations, acting on our behalf under contractual obligations; and
- Legal and regulatory parties — law enforcement, regulators, or other parties when required by law or to protect rights, safety, and security.
We do not sell consumer health data. We obtain your consent before sharing consumer health data, except where an exemption applies under applicable law.
5. Your Rights
Under the MHMDA and similar laws, you may have the right to:
- Confirm whether we are collecting, sharing, or selling your consumer health data;
- Access the consumer health data we have collected about you;
- Delete consumer health data we have collected about you;
- Withdraw consent to the collection or sharing of your consumer health data.
To exercise any of these rights, email info@teaandslippers.com with the subject line Consumer Health Data Request. We will respond within the time frame required by applicable law. We may need to verify your identity before completing a request. We will not discriminate against you for exercising your rights.
6. How We Protect Your Data
We use reasonable administrative, technical, and physical safeguards designed to protect consumer health data from unauthorized access, disclosure, alteration, and destruction. No system is perfectly secure, and we cannot guarantee absolute security.
7. Changes to This Notice
We may update this Notice from time to time. When we do, we will post the revised version and update the effective date at the top of this page.
