In the Fall of 2024 I started looking for a Memory Care Facility for my husband. He has severe Dementia. I chose Cascades Memory Care in Grants Pass, OR. It was wonderful and they really wanted to accept my husband, but they couldn’t because of his food allergies. With the size of the facility the state wouldn’t let him be admitted because they couldn’t keep him from grabbing food from other people. We were told we needed a small facility where they could keep a better eye on him. We next applied to PLC (on Heidi Lane) - a couple of them came out for an interview. When I told them what we fed him to avoid his allergies (meat, veggie, & fruit) they said that they could handle that. I provided them with special muffins for him and gave them the recipe for lemonade I make for him (and supplied the ingredients). But, they gave him what everyone else was eating. They said they couldn’t keep him from taking food away from other people (it was against the law, they said - if he wanted he could take it). Even if a resident got up and slapped him on the back, which happened. And even though two people were in the kitchen when one could have been in the living room and diverted him. Which they “couldn’t do” either. He was left in soiled clothes & bed, urine in several places in his room. They let urine stay on the bathroom counter for hours. We saw him being isolated from others and neglected. Then they put him on a medication to calm him down. When that didn’t help his aggressiveness, they increased it, which made it worse. They continued giving him a supplement he was on at home and didn’t bother to check if it could be given with the prescription they put him on. He was there for one month. It got so bad they called 911 and he wound up in the hospital for a month in a SOMA bed - trying to get the medications out of his system and finding what would help him not be aggressive. He is now in a good facility. He gets the help and attention he needs and he is doing good. I DO NOT recommend PLC at Heidi Lane in Grants Pass. One specialist I talked to said unfortunately all PLC in the area are like that. I’m just glad God led us to one that cares for him

Have had loved one here for a few months. Not a good situation. It looks like they never have cleaned room or bathroom. No supplies replaced like toilet paper, soap, etc. Charge for every little thing and do not disclose how extra costs are tallied. Laundry is lost. Poor communication. No exercise. Mail not delivered. When you ask management for clarification they tell you to call corporate office in Wilsonville OR. So disappointed and frustrated. Looking for new facility.
