UNDER NEW MANAGEMENT. My sister has lived there for years, and in that time there has always been a small, core group of caring individuals working there, but they did not seem to have the support of their management. The result was that the standard of care -- except by that handful of caregivers -- lacked accountability, and so was inconsistent. Now they have the support of a new Administrator, Anne Markwardt, and she is bringing the energy. I visit twice a week and i see she has hired a cleaner to maintain the building - he is dusting, cleaning windows, buffing floors every time I come. She has instituted Mandatory Skills Training seminars and improved the quality of the Activities for the residents, among other cultural improvements. She is genuinely interested in her work, and is responsive to my questions and concerns, every time. Her hard work is taking this facility in a better direction.

My relative was moved here recently at the recommendation of their hospital social worker. I refer to this place as the human warehouse. Upon arrival, the smell of urine and human waste greet you. Incoherent residents deteriorate in the "dayroom". The front desk is rarely staffed, so a resident or food service staff might let you in. The staff handle patients like bags of trash. The rooms are poorly kept; think of furniture harvested from the dumpster at Goodwill. The staff do a horrible job of coordinating medical appointments. Here's one example of their neglect... My relative was in the ER several times in 2 weeks for a UTI. He was finally discharged to the ER for the UTI; it was so bad that IV antibiotics were required. The staff claimed my relative became upset and tried to remove the catheter- apparently ignoring the pain that their neglect caused! They never mentioned the UTIs prior to this. This is the place of last resort- the place you hope you never have to stay in. Absolutely awful!

This place is disgusting! I wouldn’t send a soul there! The smell of urine is overwhelming. My loved one fell 3 times within 12 hours. Poor staffing DO NOT GO THERE! Zero stars

Potential resident beware before ever considering this facility for a loved one. The supervisors were unprofessional on all occasions and did not return phone calls. When our loved one was discharged to go to the ER no one in the family was notified and we had to find out from the hospital that he was even there. During the stay we visited multiple times a week and each occasion he was always without water and asking for some. We also labeled all of the clothes and belongings like we were told to do by staff and all of the clothes, bedding, and blankets were lost or placed with other patients instead. We had to ask multiple times for the items to be found and returned. In our case nothing regarding billing was addressed or discussed with us even though it should have been laid out in the terms of our contract. We received multiple duplicate invoices and had trouble even getting a hold of someone for clarification. Terrible experience while already going through an emotional time. I wouldn't recommend this facility to anyone. I appreciate your response and the protocols you have in place. Given the severity of dementia that my family member had in your care, those protocols did not take into consideration the additional care they needed and required. (He was not able to go to the machine to get ice water. He didn't even know where he was.) My hope for other families in your care is that as you investigated this case, that you saw a need for additional training and that the appropriate employees receive that training.

My Sister has been at Muskego Health Care Center for 9 years. She has been treated like a part of the family since she came. The staff is excellent and caring. The Social Workers are kind, very sincere and strive to know the residents as well as the family. They have activities planned everyday and the food is very good. My Sister considers Muskego Health Care Center her home and Loves being a part of the family> We are blessed to have such wonderful people in our lives taking care of our family
