We originally chose Beech Grove Meadows for several reasons, but four stood out most: the attentive staff, the dining facilities, the step-up care options, and the ability to customize my father’s unit. We even made a substantial investment in improvements, including a high-quality fence and a dog door installation, to make this feel like home. Unfortunately, much of what made this community appealing is no longer available. The dining room and assisted living care have been discontinued, yet rent was raised substantially—an increase that feels unjustified given the reduced services. My father went several days without air-conditioning, and nothing was done until I threatened to purchase a portable unit myself. I have left multiple messages for Administrator Jeremiah Johnson and have never received the courtesy of a return call. The staff members themselves remain wonderful, but they are clearly limited by a management team that does not support them. Only 15 months later following the difficult move from our family home of 46 years, we are now faced with the painful decision of relocating my father once again. Based on our experience, I cannot recommend Beech Grove Meadows to anyone looking for quality care, support, and open communication for their loved ones.

This is my 4th stay at Beech Grove Meadows for rehab. Upon my return yesterday several members of the staff came by to greet me and welcome me back. The facility is very clean and does not smell like urine like a lot of nursing care facilities. The staff is attentive and friendly. I have always been treated with the utmost respect. And the food is yummy!

This place is the filthiest rudest facility and made the mistake of leaving my motherinlaw here for rehab after a stroke that left her rt. Arm limp. As well as not being able to speak. After requesting her get showered after a week they lied to me said they gave her a shower she was wearing same clothes for a week they lied for a month till I went there and gave her her first shower as well as brushed her teeth as she developed open sores in mouth and tongue plus thrush ! Complaining ALLWAYS is overlooked. She has lost 25lbs now and if left here she WILL DIE.. I had the head nurse in the dementia unit actually tell me that SHE HATES ME to my face for asking her why they always lie about EVERYTHING, they made me quit trying to help her as her arm was so stiff she could not straighten it out and her hand was curling in. She is much worse shape now then when we brought her there. This place deserves a negative rating, not even 1 star. After reading dozens of the reviews about the staff here I have to say everyone has the same opinion as mine they are terribly rude and sit around and gossip amongst themselves. You won't get any questions answered truthfully as well as treated like a stepchild at best. If you truly love your elders find ANYWHERE BUT HERE to take them. I've been very truthful and "soft" as possible on this review. P.s. the food here stinks. My wife and I have cried over the mistake of not doing our homework and reading real reviews from others here. It's obvious that they write all the 5 star reviews to offset all the terrible reviews...lies and more lies

If I could give no stars I would. A gentleman who is essentially my grandfather has been transferred here by the VA hospital and is here for rehab from losing a finger. I come home from running errands (he is my neighbor) and there he is. On his front porch. Guess what he's been doing? Yup driving. GUESS WHAT IN HIS ARM STILL?! His IV! Yeah. They let him leave with an IV in his arm. I have already called the hotline after calling them well over 10 times let it ring 6-10 times and no response. Here we are an hour later and no call back. The lovely lady (I believe her name was Katelyn) who answered the phone was very attentive, respectful, and made sure all of the information was correct, verified his location, and assured me this would be investigated through corporate. I hope the nurses who let him out with an IV in his arm lose their job being as he has already been fighting infection from losing the finger. Less than 3 weeks after this review he passed away. Rest in Heaven Walt. You are so deeply missed.

My mom was in the memory care unit of this facility. I would never send a loved one here. It was a big mistake. She fell many times, one with a trip to hospital for stitches. If the employees actually worked maybe the patients would get the care they deserve. One nurse sits in her little desk area and scrolls on her phone all day, everyday. There will be only one aid doing everything for all the patients or I should say try and do it all but she's only one person. My mom would hit the call light and they would hurry and turn it off but come back at least 25 minutes later to do what was asked. One time I went up to the cafeteria to see what was taking so long and there standing around talking doing nothing. The patients suffer. My mom would sit in a wet depend for hours upon hours and develop a rash. One time a patient asked me for some hot tea. I didn't know where to get it. I asked the lazy lady in charge back there if she could get her some tea. She said she would in a minute , 20 minutes later still no tea. All the while she played on her phone. It's like a circus back there. This place doesn't deserve one star. It's dirty, smells, and most of the employees in memory care , just are there for a pay check. Please don't make the mistake we made by leaving a loved one in their care because they just won't receive it. !
