I would not recommend this facility to anyone looking for quality memory care. Our family experienced poor communication, inconsistent care, and management that seemed disorganized and unwilling to work with us when concerns arose. Instead of supporting our family through a difficult time, we felt dismissed and left without answers. It was an incredibly disappointing experience, and I truly hope management makes meaningful changes so other families don’t have to go through what we did.

Our family entrusted this facility with the care of our mother, who has advanced young-onset Alzheimer’s disease, during one of the most difficult times of our lives. Unfortunately, our experience was deeply disappointing and, at times, alarming. Despite accepting her as a resident and assuring us they could care for someone with her condition, it quickly became clear that they were not equipped to manage the realities of advanced dementia. Rather than working with us to identify the cause of changes in her behavior, we were repeatedly told she needed to be admitted to a psychiatric hospital. What is most concerning is that her significant urinary tract infection was overlooked. Instead of recognizing that a serious UTI can dramatically worsen confusion, agitation, and behavior in someone with dementia, the focus remained on sending her to a psych ward. After bringing our mother home, she received appropriate medical care and was started on the correct antibiotics. Within a short period of time, she returned to her baseline. It was heartbreaking to realize that an untreated infection—not simply a psychiatric crisis—had been contributing to the dramatic change in her condition. The communication from the facility was poor, and we often felt that we had to fight to have our concerns taken seriously. Ultimately, rather than providing the specialized care they promised, they discharged our mother because they were unable to manage her needs. Families deserve honesty about a facility’s capabilities and staff who look for underlying medical causes when a resident with dementia suddenly declines. We hope no other family has to experience the fear, stress, and disappointment that ours did. We cannot recommend this facility for anyone with advanced dementia or complex medical needs.

Clear Fork has been great for my father-in-law. He spent a great deal of time in assisted living & then transferred to memory care unit. The staff & the facility have done a great job. The residents are so amazing. When we come in to check on my father-in-law was always enjoy seeing the other residents. It a very sweet and caring place. We the family of Eddie hopes he lives out his final days at Clear Fork Willow Park. This is where he feels safe & cared for.

This place is less than 1 star. While there were some good aides, clearly the administration doesn’t have a clue as to how to run a facility. My brother's stay there was detrimental to his health physically and mentally. He never received regular physical or occupational therapy which we were paying extra for and I have pictures of how dirty his room would get in less then a day! I purchased at least 6 sets of sheets and blankets for him but all they could find was one set. One aide was very demeaning to him and accused me of being passive/aggressive, which described her not me. There were many other incidents where he wasn’t taken to eat or just left sitting for hours. Some family members were there almost every day and tried to address all of this but we just kept getting excuses and promises to do better but they did not. PLEASE DO NOT TRUST YOUR LOVED ONE TO CLEARFORK it was a terrible mistake.l believe the treatment he received there to be instrumental in leading to his death. He should have been safe there but he wasn’t.
