My mom had a stroke in March of 2025 and as a result now resides in the skilled nursing unit of Creasy Springs. I couldn't be happier with the care my mom receives there. The staff doesn't just do their jobs; they truly know my mom. They address her by name and always take the time to listen, which has made the transition so much easier. It feels less like a facility and more like a community where dignity is the priority. The medical team and nursing staff at Creasy Springs are top-notch. Communication with me has been proactive and transparent—I always am updated immediately on any changes in medication or health status. The facility is also consistently clean and uplifting. Residents are encouraged to participate in live music events, various groups of cards, puzzles, and games, and trivia competitions. The dining hall serves hot, nutritious meals that my mom actually enjoys. I highly recommend Creasy Springs for anyone seeking peace of mind concerning adult care.

Broken leg brought me here. Creasy Springs has the best food and the rehabilitation is terrific. I would not hesitate to come back should the need arise. Wonderful staff as well and my insurance, medicaid, covers the cost

I'm a patient at Creasy Springs Nursing Facility at 1750 Creasy in Lafayette Indiana. They have an excellent staff here. The nurses,Cnas, activities department, physical therapists, occupational therapists, the laundry.the housekeepers the chef here the best. I'd consider him/her 4- 5 star. All the directors work so hard to solve all issues. Extremely clean and attractive. Im throughly pleased. Thank you Creasy Springs staff for all you've done to get me this far. Sandra K. Wilson

The physical therapist are amazing they show patience and compassion and have their patients care and recovery in mind. Nurses are good and the rest of the team some could use improvement. All in all received good care.

The only reason I am giving 2 stars instead of none is because Brandon(executive director) was so very nice and fixed a couple very concerning issues. My mother has alztymers and dementia, she is also totally incontinent, has hypertension and has a lot of hip and back pain. I have been her only caregiver since 2012, on 01/23 she sufferd what the doctor said was a "small stroke" after a few days in the hospital Franciscan said she needed rehab to learn how to chew and swallow her food again. She also need intense physical therapy and speech therapy. They referred creasy springs across the street. OH GOD,I wish I would have read all these bad reviews before putting my mother there! Most of the bad reviews has and still are happening with my mom;e.g,getting 4 phone calls in one day saying my mother's last day was yesterday! The buisness lady that called was so mean and uncaring stating that my mom was self pay now; most importantly she had only been there 20days! Medicare covers up to 60-100 days. They were suppose to explain all this and have it posted throughout the building. I didn't get any of these phone calls until I had voiced my concerns/complaints. I went to visit my mother 2 days after she was admitted into creasy springs(Lafayette).I was shocked to find my mother still in bed at 9:45am trying to eat her breakfast with a STAW,she also had a soiled nighty and diaper 💩 on which you could smell in the hall. The worst part is her food was about gone so no one came to check on her or cared enough to unroll her silverware that was on her tray. I also noticed she had been in that room for 2 days with no tv.After showering her and getting her dressed it was time for lunch so I got her walker and helped her to the dinning hall. On the way the nurses looked shocked and said they have never seen her walk! I also stopped at the nurses station to let them know that there was pooh that I couldn't clean up because there wasn't any cleaning supplies in tje bathroom,(toilet seat and shower seat.I also had to go get towels and wash rags cause there wasn't any in her room. i also realized 2 of the 3 of her nighties I had dropped of at the hospital were still clean so she had been sleeping in the same soiled nighty. I then noticed she had no dirty laundry but some of her clothes were missing so I asked the nurse who then took me to the laundry room. I explained that I told the staff on her first day that i was doing her laundry cause I didn't want them tagged,We finally found her cloths with no tags? I took mom back to her room around 3:30 pm and saw that the sioled pooh bathroom still hadn't been cleaned! Went out to find a nurse but not one in sight, thats when I drove home and called to speak with someone in charge.
