Azalea Commons used to be a wonderful place. Since the staff change took place in late 2024, things went downhill. My Mother was not getting her meds as needed. We found about 3 weeks worth of her meds in her purse on March 1. Fast forward to March 23. She had fallen in the bathroom and I got a call saying she was fine, but had fallen. March 24, 2025, I received a call she was being sent to Panola Med Center for x-rays. I came to visit her around 4 that day. She had clearly experienced a stroke, and no one had noticed. Not the kind of facility you want your loved ones at. She has since been moved. The fact that she did not get her meds consistently is true. The fact that I had to have the nurse call the ambulance because she had had stroke and no one had noticed. Those are definitely not unsubstantiated.
