I appreciate this nursing home being free via Medicaid, truly, but they almost killed my father. The doctors at the ER told us that this facility provided horrible care leading to his heart stopping. Too high CO2 levels or something like that.

On the outside Pearl City Post Acute looks like any other care facility, however the care my mother-in-law received was certainly not skilled nursing. My mother-in-law is an alert and articulate 95 year old who was not treated with care and dignity. On day 6 she called me crying and begged for me to get her out. She reported that she was not being cared for. She was there for 7 days and insisted the doctor had never come by even once to introduce himself. She also reported waiting for several hours to receive scheduled pain medication. Once when she asked what the delay was, the employee told her that she was not the only person on the floor and they had 40 others to take care of. The night shift care was the worst. She said she was actually ignored or spoken to harshly by the night shift staff. If you go to visit your loved one, the care staff pretty much ignore you and look irritated when you interrupt their talk story session to ask a question. I did not ever speak to anyone who knew anything about my mother-in-law. The excuse given was “I was just assigned to her case.” Everyone I spoke to said the same thing. Once we decided we wanted her discharged, we were given the run around by Susan the social worker. She seemed more interested in maintaining control over paperwork and process than fulfilling the wishes of my mother-in-law. Her lack of responsiveness led to my mother-in-law having to stay another night. Hearing her cry about how much she wanted to leave this terrible place broke my heart. I wish you could give a Zero star rating. If you have a choice, DO NOT send your loved one here.

I've been at PCN for 79 days. I arrived needing acute care and physical therapy. I was unable to stand and had T-Rex arm reach. The RNs and CNAs were professional, caring and very skilled in patient care and rehab. I know, I was a CNA over 10 years in personal care home, in a senior behavioral dept, snf dept, medsurge dept, ER dept. I worked in a resident nursing facility, hospital facility, and mental health facility. Here, the CNAs are compassionate and hardworking in patient care. The RNs are skilled in providing medical care and in providing pain medications timely as needed. The medical activity staff are vibrant in providing activities and games for patients to compliment the medical care and physical therapy they're receiving. They encourage family visits and participation along patients in a relaxed setting. The kitchen provide healthy meals with a variety of ethnic favorites as well as a few alternative substitutions to please. The physical and occupational therapist are skilled to promote patients physical strengthing and regain their ability and agility to endure daily ADLs needed for when they leave here. The administrative staff provides needed support to make sure that your stay here is productive, pleasing and goal reaching. This is a complete package, don't you think. I did, and I walked out of there with skills to practice at home to keep what I regained. REACH FOR THE SKY...I did! Big Mahalo to everyone. ❤️ 👍🤙 THIS PLACE IS THE TOTAL PACKAGE!

If you’re looking for a rehab place to send your love ones to PEARL CITY NURSING HOME IS THE PLACE! My mom started at PCNH back in the beginning of March 2025. I met and had a meeting with the whole team from social workers, head of rehab, head nurse and an office clerk. They went over everything with us and what to look forward to. Mom started there couldn’t even sit up by herself or even stand. The rehab team worked hard, supporting and build up her confidence five days a week. About two months later mom is able to sit up, stand and walk by herself. The team in rehab from PT, OT, Speech and the Boss Lady Kacie is an AWESOME OHANA TEAM! They were always there to help, answer any of our questions and concerns we needed. I’ve never saw such a positive Ohana team that supports one another in helping us families! I also spent time with other residence at PCNH and they all look forward to going to rehab. They all enjoy rehab because of the environment, the workouts and especially the rehab Ohana! My grandmother was sent to this nursing home directly from Pali Momi and spent a significant amount of time there. Unfortunately, the care she received was appalling and completely unacceptable. During her stay, she developed a wound that became severely infected due to neglect. The wound was large and deep, yet wound care only visited her once a week. This lack of attention led to a series of serious health issues. The hygiene standards at this facility are horrendous. They do not bathe or change residents promptly after accidents, leaving them to sit in their mess for extended periods. At best, residents might get wiped down once a day, which is entirely inadequate. The neglect of my grandmother’s hygiene and wound care caused her infection to worsen, resulting in her losing the ability to walk, stand, or move on her own. She also began hallucinating and became gravely ill. Her health deteriorated so rapidly that it terrified our entire family. Eventually, she had to be transferred to a hospital due to breathing complications. While there, we learned that the infection had spread to her bones, which explained her loss of mobility. This level of neglect and incompetence is shocking, and it’s terrifying that this facility is still operating. Adding to the nightmare, the nursing home does not follow HIPAA regulations and shows no regard for transparency or family communication. If a resident requires emergency assistance and 911 is called, they have a policy of waiting four hours to notify the family. Imagine not knowing for four hours if your loved one is hurt or on the brink of passing—this is unacceptable and inhumane. We had no idea how negligent this facility was until my grandmother’s health started declining dramatically. Everything spiraled downhill, and it’s clear that they do not prioritize the well-being of their residents. Please think twice before placing your loved one here—it could cost them their health and dignity.
