I cannot begin to express the gratitude I have towards Graceful Swan in Acworth. Rahina is the absolute best at caring for your loved one. Caring, compassionate and loving. Making the tough decision of moving my mom from an assisted living facility to a personal care home as her dementia became aggressive was a tough one but Dawn (the owner) and Rahina made the transition comfortable and seamless. Although my mom didn’t always express it, I know she loved Rahina and is forever grateful for her care and compassion! I HIGHLY recommend Graceful Swan in Acworth.

My brother needed assistance after several health issues. Finding a place for him was a challenge. I was referred to Graceful Swan by a colleague. During his time at Graceful Swan, his health and attitude have made a complete turn around for the better. All because of the attention and top quality care. They have been a blessing.

We really appreciate Graceful Swan PCH and their 24-hour care of our loved one. It is a private home with 3 bedrooms for patients, each getting good personal attention. In our case we also had the Medicare home hospice service, with nurses and assistants who come several times a week. The owner Dawn is a former hospice nurse. Our loved one is very happy there.

Graceful Swan PCHs have been home to many of our clients. The small, personalized care is outstanding and we know we can count on Dawn and her caregivers to provide compassionate support to our clients and their families.
