This place should be under investigation. Medicine is not being given to patients but it’s recorded that the patient received their medicine. Falsifying documents! Physical assault by worker that was not assigned to patient, now charges pending per my family member. No video cameras in this facility which is Very suspicious. Please don’t send your love ones here. The smell in this place is Horrible all the time.

I have a family member that been here for over 3 years and the place is amazing she is doing great and moving better than before. I just want to thank the rehab department and nurses GNA everyone for an amazing job , and housekeeping for keeping the place nice and clean.

From the moment we arrived, the physical, occupational, and speech therapy staff stood out — they were professional, highly skilled, and truly committed to helping my loved one regain strength, mobility, and confidence. The nurses and caregivers were compassionate, attentive, and patient-focused. They took the time to listen, answer questions thoroughly, and provide comfort when it was needed most. Overall, this facility made what could have been a stressful time much easier. Thank you again.

My faith in God, my husband some family friends kept me going in this facility, along with 3-4 of the aides, 3 nurses, the Director of Nursing and the former activities director who really cared about the residents on the days he was there. The Directer of the Rehab center and her team were amazing with the exception of one lady who needed to retire in my honest opinion. The amount of I'll treatment residents endure was deplorable. I was thrilled to leave this facility and prayed that something would be done to bring awareness of the situations happening in this facility. It smelled atrocious too often, and there were too many instances of a couple residents being left in waste for 45 minutes to 2 hours, or those walking around on the floor without clothing. Disgusting. The meals were lacking more times than not, and I've attached examples of what I received. The many times I had to remind them I do not eat pork or oatmeal, even when we met with the Dietitian and yet, agter being assured they have it in the system, I was still give. Pork and Oatmel. I see from the pics of the facility that there seems to have been updates. I pray that is really the case, and not just staging for the purpose of advertisement. There's way too much to say on this platform. I submitted my feedback however, I never heard from anyone. Also, the death rate is shocking as well. This facility is in desperate need of a complete overhaul of the sleeping rooms, the janitorial process, the culinary services, the administrator-who was supposed to fix the blinds in the room when I arrived. I left 2 months later and blinds were still damaged beyond repair. The facility knew I was arriving on a certain day, yet they were not prepared for my arrival and the EMTs had to make the bed. I was extremely disappointing and concerning. And this facility needs to do better!!! After all, they are being compensated.

This is one of the worst places ones loved one can be at the staff is rude they are from other countries they're not attentive to the patients I hf to call the state on them. Then instead of the dietician delivering the food the nurses does it after dealing with patients ,how foul. I heard the nurse tell a patient they have to wait to be changed cause they delivering food how disgusting. I swear I don't recommend no one to this facility
