My Mother resided in Memory Care at Hamilton Trace for two years and was briefly in two prior facilities in Hamilton County before being blessed with an opening there. Cindy, the RN that oversees the unit and Retta that works with the patients and families are the most professional and hardworking leaders. Further, the LPNs and CNAs are top notch and have a special place in our family’s heart. In an industry that is criticized and scrutinized Cardon and Hamilton Trace’s Memory Care facility are top notch due to the team in that unit. Retta, Cindy and the team were with us every step of the way. 10 out of 10. ❤️

Our experience with Hamilton Trace was genuinely frightening and ultimately traumatic. Over time, we witnessed repeated lapses in basic care and responsiveness that felt unsafe for a facility responsible for vulnerable people. It was not uncommon to hear residents yelling for help for long stretches of time while staff were nearby but did not respond. There were also situations where a call button was activated and hours passed before anyone came to assist. These were not one-off moments. Family members were repeatedly forced to come to the facility in the middle of the night because care simply wasn’t happening as it should. That alone should be a red flag. Medication practices were also deeply concerning. Medications were given in ways that did not feel appropriate or well-regulated, adding to the overall sense that there was a lack of oversight and accountability. The emotional and physical impact of this experience was severe. After discharge, the trauma and complications that followed led to a hospital stay of several weeks for conditions unrelated to the original injury. Watching someone you love decline because of preventable failures is something that stays with you. Some individual staff members were kind, but kindness can’t make up for an environment where distress goes unanswered and families are left to intervene. No one should have to stay on constant alert to keep their loved one safe. I would urge families to look very carefully before trusting Hamilton Trace with someone they care about.

The food system in the rehab section is not good. Even if you go to the "cafeteria", its mostly cold and they usually dont have the items you may have circled on the food request. The employees are not helpful to guests or the clients in the cafeteria. I joined my mom and didnt know the process. No one acknowledged me or even tried to help. Unfortunately the aides are occupied for about 2 hours during meal times, delivering food or in the cafeteria. This is not good if a client needs assistance during that time, as no one is available to help you, even when you are told not to go to the restroom by yourself, or you're unable, due to surgery. The management staff is all very nice and helpful. But the day to day caregivers are notN which is sad since they are the ones closest to the patients. I would not recommend this place for rehab services.

The team in the Cherished Memories area are amazing. Retta the Activities Director is absolutely a blessing from God. They take fantastic care of my mother. She has been there 5 years now and has had the best care possible. I have had multiple family members here and have always been impressed with the quality of care and love shown to all of them. I know finding a place for your loved ones is difficult but here is the place. Thank you, Hamilton Trace, for taking such great care of my family members when we needed you to. God bless you and the staff for the work you do.

Hamilton Trace has provided exceptional care to my mom in rehab intermittently over the past few years after stays in the hospital. The staff ensures my mom is aware of and engaged in activities while caring for her skilled nursing needs. Definitely recommended for your loved ones
