Update-2024 September: After 9 months, the statement of deficiencies for Meadow Lakes rank a Zero Stars under the NC department of Human Services facility inspection and ratings website. Deficiency Reports are dated 05/17/2024 with a 85 page statement of deficiency and on 07/24/2024 with a 26 page statement of deficiency. The last star ratings of ZERO were issued on 06/13/2024 and 08/14/2024. All the proof a family needs to be wary about making a decision if their loved once should be placed her for dementia specialty care. Also, on the NCDHHS inspection website, fines for the the facility has $8000.00 for 2023 deficiencies and $26,000 for an incident on 05/17/2024. That is a extremely higher than average fine for assisted living facilities. Thankful for WSOC channel 9 news out of Charlotte for keeping the public informed for a follow-up on this facility in August 2024. The state and Iredell county need to make this facility top priority for review and onsite inspections. Update 2023-November: I hope the recent name change from Heritage Place to Meadow Lakes of Statesville indicates that the facility is under new ownership. The facility owner that bought the facility in April 2022 reduced the quantity and quality of food. I witnessed the meals for several months while my father was a resident at Heritage Place. The portions would not keep a toddler content. I cannot imagine how some of it tasted. No ice for cold tea or water. Also, the level of supplies to help take care of the residents hygiene went to below bare minimum. My rating is a 1 Star the same as the NC department of DHHS has rated this facility officially. A simple google search will show the NC DHHS rating and non-conformances reports. Take a quick read of these reports and you will read where patients have been attacked by other patients to the point of deadly harm. The state reports also show med techs not properly keeping up with medication inventory. Family members being told that if their loved one went under hospice care, they would have to find new placement for their loved one. Also, the reports will reveal residents getting out of the facility and could not be found within 15 minutes. Again, I hope with the new name change also includes new ownership that cares for the residents. Most of the personal aids who cared for the residents seemed to care but had little say on how the residents were treated or fed proper nutrition. I have laid awake at night worrying about the residents at Heritage Place after I was blessed to move my father to a facility where he gets the proper care he needs. He has not had anymore bruises, he is hydrated, kept clean and fed very patiently each meal because he can no longer feed himself. He has gained weight back. Please be very cautious when considering placing a loved one in any facility; especially this one.

As an individual that has worked in the community and the surrounding areas. I am so thankful for the change in Leadership. Myself personally that has a family member live at the community. I see positive changes and energy which enrich the life’s of loved ones and staff. The leadership in the past few weeks have shown me they have compassion care and love and only want the best for all that makes Meadows home. I can not wait to see the changes ahead as management had turned things around for the better good of the community but the Statesville area providing the upmost care. We are thankful ! Mistakes may happen. But it’s the changes that show the amazing efforts being put forth for my loved one and seniors. Many kiddos to the staff and leadership of this community.

The fact this place is on the news for all their violations is so sad. WAKE UP PEOPLE!! The lead administrator is a coward for not answering questions and/or becoming accountable. Stop fooling people. An innocent resident died at this facility. If it’s under new management, you would think they would fix all the issues. This place is a dump and truly an unsafe place for anyone!

My husband called to inquire about residence for my mother and requested some information to be sent. It never came. He called back in a couple of weeks and got an apology and was promised it would be taken care of. It still never came. He called a third time and the blame for the negligence was directed towards the office Mgr. He was promised that an email would be sent immediately. It still hasn't come. If this is how things are run there then we absolutely want nothing to do with this place.
