Beware! Resident assessment completed by facility DON, provided all necessary medical information(including med list), completed all resident documents, paid deposit/move-in fee, and given possession of apartment for my mentally disabled brother at Story Point West. After living in his own home for 30 yrs & needing more care, physical therapy & proper nutrition, we LISTED & SOLD the home. For the next MONTH we purchased new furniture, transferred most of his things to his new apartment while acclimating him to his new surroundings. Just 5 days before his actual move-in day, I was called by the administrator and told there was a "situation" and that my brother could not be accepted due to "medications" he was prescribed. Medications they were aware of when I signed the resident agreement and was given a key. The rest has been a blur and nightmare of trying to find him placement, finding immediate availability & paying numerous apartment applications in less than 3 weeks of his him turning over possession of his home to the buyers. Never in my mind did I think a facility I had contracted with would/could just change their mind!!! The disability "system" has been less than helpful, beyond stressed, confused and angry! BS AND MISLEADING RESPONSE! Two assessments are NOT required as this facility! Had I moved him in within 30 days of initial assessment there would not have been a change in plan. TWO virtual conferences with other care providers PRIOR to scheduled admission were ignored by DON & Adm who later stated they " weren't" obligated to attend". Perhaps attending would have prevented this incident from happening! Interesting that no meds or physical changes had occurred between each assessment, yet suddenly he didn't meet the criteria. SP cannot reveal WHAT changes occurred to prevent admission after 2nd assessment?? It's almost as if someone familiar with my brother/his providers discouraged admission based on personal opinion...

The staff at StoryPoint West are incredible. They are kind and caring and give you all the information without being pushy. It is a well cared for place with excellent activities. The residents are clearly taken care of and loved.

Tracy gave me the tour, and she was outstanding — kind, informative, and truly passionate about the care they provide. She took the time to answer all of my questions and made me feel completely at ease. They also have a wonderful memory care program that is thoughtfully tailored to meet the unique needs of each resident. It’s clear the staff are highly trained, compassionate, and dedicated to helping residents with memory challenges feel safe, respected, and engaged. I really love StoryPoint Fort Wayne West. I am so glad we chose this community for my dad. The staff has been nothing short of sweet ,kind and very caring. I am forever grateful for their knowledgeable and kind staff. They have this thing called 1440 and they really do care for their residents in that manner. Everyone is always helpful &kind!! we could not have picked a better place for our dad!

Our mother has been a resident in memory care for two years. Our number one goal was for her to be safe and taken care of. She is that, here at Story Point. If there is/has been any bumps in the road or concerns, the staff has always listened and have done the best they could to resolve them. No where is perfect but we believe we found the perfect place for our mother to receive all the needs we believe she deserves. She is happy and comfortable. That says it all.
