My mother has been there for several years. At first it was an amazing place. Mom was safe and comfortable. But just a short time ago, the facility had gone down hill. The smell is so bad throughout you don’t want to breathe. The bathrooms (men) dividers was extremely rusty and half the stalls was out of order. The shower and toilets were covered in poo. And yes I have photos and sent to the administrator. Missing exit lights from the ceiling and all that’s left is wires hanging down. They started shutting off the hall lights at night and residents were falling and getting hurt while trying to go to the restrooms. The staff gave my mother another residents medicine and my mother had to be hospitalized as it was an opioid. So many other tragedies to include. As we tried to contact the administrator (Rhonda), it was minimal or no response at all. We are still trying to retrieve information about my moms financial records and receipts since September of 2023 and we keep getting a run around. Found out there was foul play in their other facility they had to shut down. Fraud. We just moved my mother to an amazingly clean place In Marion and the staff is actually nurses and have the knowledge of how to treat patients (residents). My suggestion. DO NOT USE THIS FACILITY. !!!!! It’s a run down place that needs to be bulldozed down. It is not safe there anymore. Staff looks unprofessional. These photos was taken weeks ago and nothing has been done about it. Visited several times and the conditions are still the same.

Grandmother had mystery liquid on her feet, fed her corn dogs as a diabetic patient, then overdosed her insulin twice. If you love your folks, find better or just keep them in the shed out back. Also didn't like people checking that they did their jobs, so make sure to check up on your folks! If you don't visit they will not do their jobs. DHEC should have shut this down along with Hannah Manor a long time ago.

My mom stays there she seems to enjoy it my aunt also stays there and she seems to enjoy herself to they are friendly staff and will help you with your family in any way they can I would recommend Johnsonville. Like here for your family member
