My mother has been a resident for the last year at St. Croix Health and Rehab. The facility is beautiful with large private rooms for each resident. The staff have been AMAZING and provide the best care and support possible. I feel fortunate and can rest easy knowing my mother is well taken care of. Highly recommend to anyone looking for a long term care facility for a loved one in St. Croix County area.

This facility is very clean and well kept. The residents are greatly cared for and the staff is well qualified and thorough in the care provided. The rehab team is wonderful and the Long Term Care rooms are spacious and set up well. If you have need of a place to stay for rehab post surgery/ hospitalization or as a long term option, I would strongly recommend St. Croix Health Center!

Our family member is moving from a group home in MN to the St. Croix Health Center. Trying to navigate changing her services and SSI from MN to WI on our own was stressful and daunting. We met with staff today that walked us through everything that needs to be done. We left with a list of 3 tasks to take care of and a tremendously lower stress level. They are taking care of the rest. Needless to say, we are thrilled with the staff! The facility is in pristine condition. There is a full range of PT equipment, billiards, planned activities, and a salon. All rooms are private, adequately sized, and able to be personalized with a built-in knickknack shelf and the ability to put up pictures on the wall. Each room has a private bathroom with wheelchair accessibility, handheld shower, and built in seat in the shower area. Not only are we relieved to have found this facility, but we are excited for our loved one to be moving here.

My 97 year old Mom, Anne suffered many indignities while living at St. Croix Health Care Campus in the Longterm care. They were always short staffed and had poor communication with the different shifts of nurses, and CNAs. My Mom lost over 30#s in 6 months, had bruising of unknown origins, got a pressure sore on her toe, had pneumonia, a UTI and had an eye infection. I installed a video camera in her room and saw videos of CNAs without gloves performing peri cares on my Mom. My Mom was often left alone without any comfort, care or attention during the 3-11 shift. The State Department of Human Services - Division of Quality Insurance conducted an investigation and found sufficient evidence to confirm violations of state/federal laws. My Mom was denied basic human dignities such as routine toileting, her preferred foods, and compassionate care. There are so many administrators here that the place in theory should be run well. We found the social worker Lisa Lehey to be condescending, uncaring, and downright disrespectful. The place is an absolute disgrace. Most people who live in this facility have little to no financial resources and the ability to move to a private facility. My Mom being one of those. She was on Medicare and State of Wisconsin Medicaid . My Mom having dementia, Alzheimer’s disease, advanced age, unable to speak, walk, was not a good combination for being in this facility. My Mom died in the facility and until I die this will be one of the biggest regrets of my life entrusting her care to them.
