I had the unfortunate experience of staying at Pathways Adult Living in Fort Worth, TX and I cannot stress enough how unsafe and poorly managed this place is. From the moment I arrived, it was clear that resident safety and well-being are not priorities here. This facility claims to provide assisted living with care, but my time there was filled with horror and negligence that put my health and life at risk. First and foremost, the lack of security and oversight led to repeated incidents of sexual assault. Every single night I was there, I was subjected to unwanted advances and assaults by another resident who had unrestricted access to rooms. The staff seemed completely oblivious or unwilling to intervene, despite my pleas for help. There were no proper locks on doors, no night patrols, and no real protocols for handling such serious issues. I felt trapped and violated, and it’s appalling that a place meant for vulnerable adults allows this kind of environment to persist. On top of that, the medication management was a complete disaster. I rely on life-saving medications that need to be administered on a strict schedule, but they were never given to me on time—if at all. Doses were missed, times were delayed by hours, and excuses ranged from “short staffing” to “forgotten logs.” This isn’t just inconvenient; it’s dangerous and could have led to severe health complications or even death. How can an assisted living facility fail so badly at the basics? The overall atmosphere is one of chaos and indifference. The building at 921 West Cannon Street feels outdated and unclean, with little attention to hygiene or resident comfort. Staff turnover seems high, and the ones who are there often appear overworked and untrained. If you’re considering this place for yourself or a loved one, run the other way. There are better options out there that actually prioritize safety and care. I left as soon as I could and reported my experiences—stay away!

My nephew lives at Pathways Adult Living, and they've been an absolute godsend for our family. The staff there truly cares for their residents, and they're committed to providing the best possible care and quality of life. My brother and sister-in-law are incredibly grateful for the exceptional care Pathways provides.

We have had our adult son there for over 10 years and we are so grateful for Ms. Linda and all of her staff.Ms. Linda genuinely cares for each and every resident. We would be lost without Pathways.

Absolutely disgusting facility. Staff is incompetent, emotionally and possibly physically abusive. Stay away.
