We've had the best experience with our loved ones at Azalea Commons. They care for our family like their own. Blessed to have a great place to send our family to.

The administrator basically kicked my mother out because we complained about the awful attitude of one of their nurses. They have ONE nurse on day shift only for the entire facility of up to 74 patients with NO nurse on night shift!

This facility is very poorly managed and very inconsiderate of the residents as well as the family members. Nurses and aids have very bad attitudes and are backed up my the management. The meals are inedible and portioned for small children. I really feel the state should investigate

DO NOT SEND YOUR FAMILY MEMBERS TO THIS PLACE THE MANAGEMENT IS TERRIBLE. PHOTOS,VIDEOS, and AUDIO I CAN SHARE. Something needs to be done! SUCH SELFISH PEOPLE OVER THAT PLACE.

Do not send your loved ones to this facility my dad lives at this facility by his choice and he is not satisfied at all . The food is not good often there and not enough food to feed everyone so they only allowed half a serving spoonful, if they need a special diet they will not fix the proper food. We send him care packages each month so he would have food to eat. The staff goes through his personal belongings when he leaves his room. No matter what the reasons a person may live at a facility like this they should be respected of their rights.
