Do not send your loved one to this facility. My mom, Joanne, was left in bed continuously and the facilities’ excuse was that she refused to get out of bed. I told the facility repeatedly to call me if my mom refused services and no one ever did. I have many photos of feces leaking out of her brief and dried on. I even spoke with the staff doctor and requested a barrier cream because her bottom was beat red on números occasions. My mom went on to develop pressure ulcers on her bottom and the facility had nothing under her to prop her off the sores. Also I went to visit my mom mid January 2026 and her urine was very dark. (She has a super pubic tube). I asked Aki, the nurse, to get an order for a urinalysis. I visited the next Sunday and found out that no urinalysis order was given. At this point my mom was very ill and had to go to Advent hospital ER where she had a severe urine infection and had to be treated for that. Then on 2/15 I went to visit my mom and was told that she had vomited on 2/14 and had not eaten all day. No one called to tell me that my mom was ill. My mom was complaining of stomach pain. The nurse said she didn’t think my mom needed to go to Advent ER but I insisted she go. My mom has now been in the hospital several days with impacted bowels. Apparently she had not had a bowel movement for days and Shawnee Gardens had done nothing to rectify that. The facility is also dirty with the clothes I have bought for her having disappeared. A handmade blanket from Ecuador that I bought my mom is missing even though it had her name on it. Her tower fan is missing too. None of the staff seem to know what have happened to these things even though everything has her name on it. My mom was also repeatedly left laying down in bed to eat. The facility has notated that she has swallowing difficulties but she was still given food lying down. My mom was never bathed and the facility says that she refused showers. I requested that she be given bed baths and this never happened either. My mom hasn’t been bathed in years and no bed bath. My mom’s hands, face and neck are filthy when I go to visit her and I clean her with clean wash clothes because the staff never does this. Also, my mom never has water in her room. During a care plan meeting an employee told me that my mom could go get the water on each floor. My mom is bed bound. As a result my mom’s urine is always concentrated and a dark color. It’s a wonder that my mom hasn’t died at this facility from the poor care. I could go on and on with more examples. I have tried speaking with management over the years expressing my concerns, but the care never improved. This facility deserves a 0 rating. If you love your family, do not place them in this facility. It is the worst.

I was glad I made a 2nd visit during its lunch hour before mom went there... and saw how terrible the food was. Not a single person helped any of memory care patients. They didn't even care I was there to check out their services. Three staffs were playing phone and chatting during the entire lunch. Mom needs help on everything for her daily living. I did find a nice place for her but had to wait a little longer. It is worth of waiting sometime. I totally understand someone could be starving at this facility.

This is the worse long term care facility I have ever seen. My sister in law is there and she is in hospice for severe protein calorie malnutrition- translation, she is laying in their facility under their care as they have literally watched her starve almost to death. She was 480 lb 3 yrs ago and now she weighs 160 lb. She has gained 8 lb since we came back into the picture. Her husband died March 7 of this year and he was the one acting as her guardian. We had no idea she had been laying in bed in that place being starved to death. When I asked the nurse why she was on hospice, I was told it was because she would not eat. That is not at all true. We have since been there daily to feed her and she will eat whatever you feed her. She will drink when offered a drink. The reality is that because they have not been getting her out of bed, it requires someone to be at the bedside to actually feed her. They are the ones that have let her get to the point where she is too weak to feed herself. She was helping to cook dinner before she went in there and certainly was able to feed herself. She was with us in our care before her husband put her in there. When she was at our house, she was cooking wonderful meals just 2 years before she went there. I have walked into her room and an untouched tray that is not even in her reach and is still covered will be sitting on her bedside table an hour and a half after the meal has been served and then someone will just come in and pick up her tray. Obviously no one there can figure out that when someone is losing mass amounts of weight but will eat when being fed, that person is consistently not being fed. Numbers do not lie, she has been regaining her weight as soon as we began being there daily to feed her ourselves. The administration there is making us go to court in order to move her because they say she cannot make her own decisions. I’m sure the real reason they do not want her moved is because they know the next healthcare provider she gets in front of is going to lose it and arm us with the documentation needed to take to the state. She has told us that she wants out of there. My husband came into her crying this week and when he asked her what was wrong, she said “they will not help me”.I have not even got to the smell of the place. Her entire room more often than not smells strongly of urine and stool. If you are considering this facility for a loved one, I would definitely recommend against it. My sister in law does not have a terminal illness that has caused her to be on hospice, she has just been starved. Prisoners in this country that have committed terrible crimes receive better care than she is receiving. I wish I could give this place negative stars.

My brother was in this rehabilitation center starting mid October until last night November 7, 2024 when they put him out not caring if he made through the night or not! My brother suffers with mental health issues and is a new amputee and lives alone, this is a new life for him! The rehabilitation center discharged him with just a wheelchair and no meds at all but pretended to care if he still had an infection and his overall health in which he was being treated for the last 3 weeks at their facility. The medical transport just dropped off at his apartment late last night, I’m not blaming them they were just doing their job! Most rehabilitation centers will discharge with meds to last until the Primary Care Provider sees them (11/13/24 and the knew this). I went to the rehabilitation center 3 times this week to ensure a smooth transition and discharge and to make sure he had everything he needed to survive at home. I stopped by his apartment this morning when the Home Health provider was there who asked about his medication. She was shocked there were no meds at all that he desperately needed. We had to scramble to find someone to write the life saving prescriptions and send them to the nearest pharmacy. The prescriptions antibiotics, insulin and 2 other prescriptions weren’t ready until 3:30pm today and can’t start them until tomorrow, he went all day without his medication! This is totally unacceptable they don’t give a damn about your family members! The Social Worker is a whole hot mess too! I would not recommend this facility to anyone! I will also file a complaint with the Adult Protective Services for abuse and neglect!

I've been here for two months. It has been very difficult, the administration elected to place a very difficult person as a room mate , noone wants in their room because you can't sleep with all the problems she causes. The administration would not listen to my complaints and was not helpful in doing anything to help me have a better stay. I had to go to legal action to get any movement for help. For 2 months I've told the kitchen I can't eat certain foods and and yet I get it all the time. I received only 3 showers for the period of 2 months here. I was happy with the nurses and aids I had. They are overwhelmed with the work. But try to care. The rehabilitation department hasbeen very helpful getting me stronger so that I could go home. I'd rate them 7out of 10.nursing staff7out of 10 ,diet department 4out 10. If you are searching for a rehabilitation facility i think there are better ones in this area. I will not return to this place.
