I just want to say that when I first arrived at the facility, I wasn't sure if it was going to be a good fit for my mom who was needing rehab, after a lengthy hospital stay. Well, within a few hours, I knew that she was in the right place. All of the staff were so good with her, and she didn't have to wait long to have a need met, which is what you want for your 94 year old mom. Food is important to my mom, and she loved the food. It was all of a homemade type of food and every meal arrived hot and was very good. That's important when you are recovering! The first night she arrived, Nurse Jennifer was preparing to get her all checked in, and realized the time of day (late evening) and knew the kitchen would be closing soon for the day. She put everything on hold and made sure my mom had a hot dinner before moving on to getting her checked in. The physical therapy and occupational therapy was also good and they worked with my mom 5 days a week, and sometimes on the weekend too. The only thing is after PT & OT patients are returned to the room and unable to move around without help for their own safety, did I mention she is 94. I guess I would say don't judge a book by the cover, SLSCR is clean, but very dated, ( my mom said the bed was very comfortable, just old like her), and could use some upgrades, but I will take an older facility with caring staff and good food over a newer facility and staff that doesn't care for your loved ones needs. After two weeks I decided it was time to get her home, and they helped get her set up for home health care for continued rehabilitation, which went very smoothly. She is now recovering even more, but if she needs rehab or nursing care, I will definitely look at SLSCR for her needs.

My Mom came here after a week in the hospital. I chose this rehab because it got 5 stars on Medicare review and Yelp review. The main concern for me was patient/staff ratios. This is a small facility where everyone pitches in where and when needed. The staff all knew my Mom's name and cared about her. Tge food is all homemade and not from Cisco. I cannot recommend South Lyon Senior Care and Rehab more highly.

Our experience during my mother-in-law’s stay was extremely distressing and unacceptable. She was transferred to this facility for short-term rehabilitation following a fall with injuries. She has moderate dementia, and the staff demonstrated little to no training in caring for a patient with cognitive impairment. Despite extensive communication with the care team to establish a plan based on her abilities and needs, this plan was not followed. We live out of state and left after day two, at which time she had still not received basic hygiene care. We were told this would begin as part of her therapy plan. When we returned seven days later (nine days into her stay), she was in the same clothes, her hair was unwashed and uncombed, her dental implant was impacted with food, and her overall condition was unsanitary. The toiletries we had provided remained unopened, and both her room and bathroom smelled strongly of urine and stool. Staff stated that she “refused to shower,” despite clear, documented instructions on how to appropriately approach hygiene with a patient who has dementia. This was not an isolated missed shower, but a prolonged lack of basic care. As her legal guardians, we specifically requested daily care and immediate notification of any issues. Neither occurred. A disposable cell phone we left for daily contact disappeared the day after we left. Multiple personal items also went missing, including a relatively new pair of prescription glasses essential for her vision. Staff acknowledged seeing them but could not account for their loss. CNAs were frequently inattentive, often on personal cell phones, and provided minimal assistance. Prescribed pain medication for her fracture was not administered consistently; medication records confirmed that only a few doses were given until I escalated the concern. A staff member also shared serious concerns about the therapy supervisor’s interactions with my mother-in-law, which I personally observed to be cold and inappropriate. Additionally, the facility failed to complete required financial paperwork at admission and later threatened collections despite Medicare and supplemental insurance having paid the majority of the bill. I have worked in healthcare for 30 years. These are not minor issues—they represent a pattern of neglect, poor clinical practice, and lack of accountability. The level of care provided was unsafe and undignified. If your loved one has any cognitive impairment and cannot reliably advocate for themselves, I strongly urge you to avoid this facility.

The Rehab unit (completely seperate from the company itself) was awesome, hands down ! The Kitchen staff were great, very attentive and kind, although sometimes the food was cold by the time it got to the room. Reception was horrible, they miss many calls for lack of being at the desk apparently. Its quite outdated and the ceiling leaks in multiple places whenever it rains, so they put buckets and caution signs all over the dining room instead of just fixing the roof. Maintenance is almost non exsistent when you really look around at things. Poor air circulation and its quite dusty. The nursing staff is nice and informative. However, the CNA's are the absolute worst ! VERY nasty attitudes, lazy, rude, on their phones most of the time you see them or having conversations unrelated to work or patient care, showers are always late, patients are left in the bathroom for long amounts of time waiting for someone to answer the call light, but a CNA would be sitting right next to the nursing station playing on their phone. Some patients fall asleep in the dining room, which isnt a big deal, but why not take them to their room so they can sleep properly and comfortably? The Family Services guy is very kind, informative and hands on. Communicates very well and is there to help make a difference, thumbs up ! The management team doesnt seem to ever be on the same page. They always look confused when you talk to them because they dont communicate with one another so they dont know what youre talking about. Overall: 1-2 stars simply because the nurses really just pass meds (as they should), but the CNA's are actually the ones (supposedly) taking care of our loved ones all day everyday, but they're horrible, neglectful, people, Eeek ! My aunt changed rooms once but in both rooms the bathroom always smells like urine. They would get 3 stars but the CNA's seal the deal for 1 hard star, if i could do 0 I would have.

South Lyon Senior Cara & Rehab is the only place we’ll send our Mom if rehab is needed. It may be outdated and not fancy inside but the staff are far better than any rehab facility in the area. We’ve utilized three different fancy Novi Rehab locations and our mom had a nice room but did not get the care she needed and in one place almost perished. The South Lyon Rehab PT/OT staff are fantastic and are invested in their patients’ rehabilitation. The doctor, nurses, nurses aids, and social worker Matt really care about the patients, too. If you want your loved one to thrive and return home I highly recommend SL Rehab & Senior Care.
