My Dad, John Flynn, spent his last days in Southgate Senior Living. After touring 6 facilities in San Antonio Texas, I would never consider any place other than Southgate. The staff was caring and attentive to my Dad's needs. The staff determined that my Dad was declining fast and notified me in time to travel from San Antonio and spend his last few hours together. I can never thank Southgate enough.

5 star rating for cleanliness, friendliness, basic assistance from CNA's and residents that watch out for each other. They allow cameras which was a big deal for us living away and I'd never go anywhere without them. They confirmed she was treated well. Activities, and trips were top notch. They even reminded her if she forgot about her favorites. They have an onsite LDS branch with non denominational Bible study. She loved the monthly weekday Relief Society. Maintenance was always very quick to assist or make repairs. For someone totally aware I would give 5 stars because the resident could monitor their own care (minus food issues). 1 star: The (previous nurse) administered new, prescribed/available meds but did not watch for results which we believe caused a terrible face plant fall. The NP confirmed to us he changed medicines but it didn't get passed on somehow. We never found where the break was in communication but we had repeated issues with changing meds. After the bad fall, they tried to administer the family-caboshed (and doctor) med at least four more times. The new (doctor and family approved) med couldn't be given because of a mess up in the records or they couldn't locate it. That was a real issue for us although daily meds were given to perfection. The food is another issue. Guest prices were previously great so an occasional off meal could be overlooked. They state they are working on it. We wouldn't eat with her anymore because of price and quality. I brought in my own food in, often sharing with Mom. Lessons learned: They require a big deposit (typical), but then you must also pay for an extra month when you leave (30 day notice with no exception, even with health changes they admit they can't handle). If you die, that's not enough notice but you only have to pay for half a month. (Which might be okay for grieving family.) Needing more help than they can give is not an approved waiver to the extra months charges. (Do not know if this is typical, but really fired me up as they admitted they couldn't care for her. We felt it was just a money grab because other than picture nail holes we were out in 2 days leaving a few days left in the month) Just BE AWARE! Your loved one will be in danger for a month OR you pay the monthly fee. We felt Mom would be dead in that month or possibly sooner had we left her. They tried to charge full coverage to snag a little more Mom had dementia and we did all we could to keep her out of memory care, mostly because she was still very aware of memory care group setting visuals. She was too aware to tolerate that. We had Alexa announce activities, called often to remind her of meals and another reminder for favorite activities. I made maps for directions around, with their permission, took photos of staff and residents which were labelled with names so she felt she knew people. But they simply can't handle higher needs memory-declining people even with the sliding price scale in assisted living. Mom began needing serious mobility care a week ago and is now in memory care somewhere else. For safety reasons we couldn't give 30 days notice. $$$. We put her where she had desk monitoring as she had become a nightly fall risk (with two nights of a bad fall). We should have moved her sooner to a smaller facility or less patient-CNA ratio but she had made some amazing resident friends and loved many of the staff. We believed she only had one more move in her or we would have pulled her out months ago. As a rehab place or assisted living for the capable I would recommend. Other comments left here were true for us as well. I would never again come here with anyone who cannot hide their dementia unless they are happy to be left alone. Their assisted living just isn't the place for advanced dementia although still aware enough to not be able to handle memory care. Go smaller, more intimate (but then you will lose daily group activities). It's a fine line. Good luck and best wishes with your decision and journey.

Hi 👋, my name is Penny Schiess. I just moved out of Southgate. I moved to be closer to my family. They would travel 9 hrs.to see me. I was very sick 😫 & couldn't be left alone anymore . I won't lie to you.It was a hard 7 mths. I kept mostly to myself. But like anything new,you have to try. I love people & am a real people person.I was miserable. So I decided to speak to everyone. Pretty soon I had lots of friends. Guess what ??? I became happy.!!! This is a very special place.There is a friendly feeling as soon as you walk in the door. The food is good-to excellent. Depending on who is 🍳 cooking. Pick wherever you want to set. CAs with any place you have great food & not as good 😋 most everyone is pretty satisfied. I myself am a cook,so l'm picky. The activities are fun. Candy bar Bingo.every Thursday. A group of people known as the Lunch Bunch. Gets together on the 1st Monday of the month.just sign up & you're in. We've gone Crackerbarrel ,Applebee's, Olive Garden, just to name a few. We to Walmart or Smiths every Monday.We even go to Mesquite. You don't have gamble just eat at the Buffet. We have a Hair Salon & Nail salon.You get your room cleaned every week your trash out multiple times a day. But best of all you get to bring your dog. Daylin has been wonderful through the process of planning before, during, and after my dad moved in. The staff and nurses are pleasant. I searched many places before making decision and I am happy with the choice of picking Southgate. The entire staff is warm and caring. The facility is clean and tidy too. The menu has specials for a nice change up as well.

We needed last minute access to assisted-living. We are from a different state and were visiting with my father in the rehabilitation center in St. George. We started looking quickly and found Southgate as one of our visits. We were immediately impressed with Daylin and her kindness and approach as well as the rest of the staff that we met. It is a wonderful facility with a great atmosphere, kind, and caring staff, and a tremendous approach to the well-being of each resident. Even when our time with Dad there was short-lived, the staff continued to be very helpful as we put him on hospice for a few days at Southgate. I would recommend this great place for any individual or individuals who would like to have personalized, loving, and kind care in a wonderful environment. It was certainly a blessing to my father and to our family!
