As a former healthcare administrator, executive director and COO, I can say decisively, this is one of the most poorly managed facilities I have ever visited. Arguments between staff erupt regularly, the charge nurse is verbally abusive to staff, residents and visitors, and the administrative staff is all but absent from the facility. Med passing practices are incredibly lax, and residents regularly receive other resident's doctor prescribed meds, which is and egregious and highly dangerous practice. When such errors are made by the staff, they are NOT logged as a med error, nor reported to the state when there is a change of condition in the Resident as a direct result of the med error. In my past experience, this could result in the death of a resident, and the state needs to step in immediately and ensure that corrective action is taken, along with retraining the staff on proper Med passing procedures. Meal plans in the past have been very poor in nutritional balance and quality, but thankfully a recent rehire is ensuring much better quality food for the resident population. Having toured, inspected, and provided oversight of hundreds of senior living facilities nationwide, I give this location one out of one-stars, which may be on the generous side.
