SYLVAN CROSSINGS IN WESTSHIRE VILLAGE

I had to call the facility of my mom's passing. Nobody informed me about it. I called the facility myself. Plus inappropriate pictures were taking of her ....she was in her 90s. Her Samsung galaxy phone was stolen and her Louis Vuitton brown tote was stolen. This facility is good place is good for Government run facility

The staff is wonderful, patient, and caring. They do a lot of work keeping folks safe. They certainly put up with a lot from my father! The facility is a nice environment with lovely decor. The neighborhood is also pretty nice with a pond and senior residences but also a Kwik Trip folks with some mobility can get to.

I am very disappointed not only of this facility but in myself for not doing more research into Sylvan Crossings/Touchstone Senior Services LLC, the staff for the most part were very kind but I seen a problem that most if not all residents spoke english and many of the staff could not and needed to use electronic translators which only frustrated residents, there is little to no activities, the food if you did not eat in the dining area and was brought to the room was usually cold and actually the food is left with a lot to be desired, we witnessed questionable activities by staff and between staff, when there was a VERY serious health issue with our family member we were not even notified by the facility, I have a very big issue with Touchstone Senior Services LLC for obviously ignoring the problems at their facilities and the seemingly incompetent management, when we met with the relations director for their facilities she told us everything we wanted to hear which I know is her job but I should have stuck with my gut feelings of knowing we were being taken for a ride, all in all unless there are major changes in the facility you should not consider Sylvan Crossings Westshire for your loved one. thank you.

This place started out very well & the director had to take a leave of absence & it all went downhill since. No activities or very rare, the food looks like a two-year old made it, always out of food, no snacks provided at times,staff bearly speaks English very frustrating, they ignore our requests. I would not recommend this place at all!!!!

So many issues: Food (never very appetizing in appearance) placed down but no help or encouragement to eat, missing personal items even when labeled, half the clothes and linens not theirs, order meds without notifying family of illnesses, frequent resident falls, significant cost and rate increases, constant manager and staff turnover, not following COVID protocol, contradictory reports from staff members, lack of care and dignity to loved ones during stay and after death, zero respect or compassion to family. Check the numerous violations on the state DHS website.
