Autumn Oaks is a small facility with warm and attentive care providers and residents who care about one another. Staff members interact with all the residents multiple times throughout the day and can recognize quickly if a resident needs additional assistance. The home cooked meals are balanced and plentiful, and there are various activities for residents to join. The care team is as warm and welcoming to a resident’s family members as they are to the resident. This was truly a great choice for Mom.

Autumn Oaks has a lovely staff, caring people in charge of each program, and take time to know each resident. They were certainly a blessing in caring for my mother and in allowing her to age in place until she passed at 91. The administration is receptive to special dietary requests, visiting hours, and support

Autumn Oaks has been a blessing to our family. My mother-in-law has thrived since moving in over a year ago. She has become much more social and energized being in a facility that allows her to enjoy the company of other residents and staff. It is a small, tight-knit facility with caring administrators and caretakers. The residents are not treated like just a number, but are more like part of a big family. I’ve watched as the residents help each other and look out for each other, just as much as the staff does. We could not ask for a better situation for her.

My sister was admitted to Autumn Oaks approximately 5 yrs ago due to dementia onset. Initially, her care went smoothly but over time, we observed concerning changes in the facility. Notably, communication between staff and family regarding my sister’s well-being was deficient. Additionally, frequent staff turnover led to inconsistencies in administering her daily medication. Often times pills were found on the floor! Just recently, my sister was admitted to the hospital twice within a 5 wk period with severe dehydration. This condition resulted in some unusual behavior problems. Previously, she hadn’t exhibited any behavior issues, consistently displaying happiness and contentment. Before her release from the hospital, the family received an email indicating immediate discharge from Autumn Oaks due to behavioral problems. There was no consultation with the family just a very insensitive email lacking compassion or empathy. Autumn Oaks is unfortunately our past and now we are focusing on a facility that will meet her needs. Debbie Hoffman
