EVERGREEN ASSISTED LIVING FACILITY

My Grandma been sick for two weeks During the month of May, because the care givers at this facility let my Grandma Fall. she also had a bad UTI Infection from all the soda and Tea they gave her and no water. She also had sepis. There were a lot of others problems my Grandma had going on as well. She was put in the hospital. No one from this Facility notified me nor my Dad and let us know my Granny was sick or in the Hospital. My family and I had to find out my Grandma was in the hospital ourselves.. Some lady name Denise call and just told me a month later after I already knew, So all this time my Grandma been in the hospital all alone, Sick, and Suffering. 😢 I express my concerns and ask her why didn't anyone call us because they had my number prior before my Grandma became ill. She told me she don't have to call and tell us anything by law because we wasn't her Guardian. The lady Denise told me the only reason she call me to today because my Grandma Guardian ask her too. That's Wrong, Unacceptable, and Unprofessional...and also Not Fair We are still family and should know what's going on with her. I feel like they are the blame because Employees wasn't watching, nor taking care of her the correct way they supposed too because she fell and hit her head and things start going down hill for my Grandma. THE HOSPITAL SENT HER TO PALMYRA HOME AND MY GRANDMA RECENTLY PASSED ON MOTHER'S DAY. My Family is Heartbroken!!! Also devestated. 😢

Mother in Law not taken care of as promised. Problem with this place is the person who's running in. She has no time for anyone except herself. Makes up all sorts of excuses to avoid having a one on one meeting, about some major issues.

Great Place! My aunt has been there a short period of time and she really enjoys it. They had a Valentine Lunch today for residents and family and it was very good. I have only had problems with one employee and I really don't care for her but I don't see her there that often so I try to avoid her. Over all everyone is very nice, friendly and caring. The food is good and the facility is very clean and neat. I would definitely recommend this place!

Relatively small facility. Each resident has their own room with bathroom . Large Central Area encourages residents to associate together. Well maintained and clean. Controlled access , with separate controlled access area for Alzheimer's patients. Staff knows patients by name, usually learns to recognize family members as well. Cost is about one-third of a full nursing home facility.
