This facility and the Oaks organization lack caring staff and competent leadership. They are extremely unprofessional and have no systems in place to address issues. Our mom was a resident for over 2 years and things got worse over time. Each time we raised concerns, we were met with false hope that things would get better. She was often left soiled for hours and unattended, amongst many other issues. They only want families who are not involved and don't advocate for their loved ones so they can get away with poor care. I would not recommend sending your loved one here unless you just don't care about them.

The Oaks at Conyers has been an answered prayer for our family. We relocated our 93 year old Mother who was living on her own in another state. From the intake with Ms. Hollie to the actual move in everything was seamless. The management team is caring compassionate and truly listen to their residents and families. With on site personal care, medication management and activities our mother said that this far exceeded her expectations! Under the direction of Ms. Keisha this is not a nursing home or an old folks home but a loving environment where relationships are formed and peers are respected and thrive. It is amazing to see the dedication and joy that the staff give to the residents. We love and highly recommend The Oaks at Conyers!

I would not recommend sending your loved one here unless you absolutely do not care about them. The executive management of the Oaks organization, as well as the so-called leadership at this location, only want residents whose families are uninvolved. They thrive off this dynamic so they can continue operating poorly without accountability. If you raise valid concerns or ask for reasonable changes to ensure your loved one is safe and cared for, they retaliate by discharging the resident instead of addressing their lazy staff, nonexistent leadership, and poor operations. Their Regional Director of Operations ignored our calls, emails, and texts for over a year. When she grew tired of hearing about our legitimate complaints, she discharged our mother claiming they could “no longer care for her”—when the truth is they didn’t want to be held accountable (and we have the documentation to prove it). The irony is we were already planning to move her, but it’s the principle that matters. Our mother was a resident for just over two years, and during that time, we raised numerous issues, including: 1. Chronic understaffing (many shifts with only one care partner). 2. Our mom being left soiled or wet for 12+ hours. 3. Her room not cleaned for months at a time. 4. Long stretches with no activities coordinator, leaving residents idle. 5. Months with no nurse or wellness supervisor on staff. 6. No air conditioning in her room during the summer—only fixed after repeated complaints. 7. A dirty, smelly carpet throughout memory care that desperately needs replacing. And the list goes on. This year was especially bad. The “Executive Director” was on leave for months, and Oaks corporate failed to assign anyone to oversee the facility. The Conyers location is treated like an unwanted stepchild, left to operate on autopilot. The result is chaos. Most, though not all of the staff are lazy, unprofessional, and completely unsupervised. Several bring their children to work, sit on their phones for hours, and have attitudes when asked simple questions about residents. Even when the Executive Director is present, these behaviors persist because there’s no accountability or consequences. The most egregious incident occurred this summer when a staff member with a terrible attitude falsified my mother’s medication record, marking that she administered meds on time when she actually gave them three hours late, and only after I showed up with questions. Instead of firing her for this serious violation, management gave her a brief suspension and let her return to work, putting other residents at risk. She was still employed there when my mother left the facility. The Marketing (Sales) Coordinator is friendly and will tell you everything you want to hear to get you to move your loved one in, but much of what’s said is untrue. The facility often smells like urine and feces, likely because residents aren’t taken to the restroom regularly. They claim to check on residents every two hours overnight but this NEVER happens. We know because we had a camera in our mother’s room, which they knew about. Leadership on-site is nearly nonexistent, and corporate and regional management completely ignore families. The only shining stars are Melissa, the new Activities Director, who is wonderful, and a few longtime staff members who truly care about the residents (Pauline, Jennifer, Ursula, Tee, Lou, Charmaine, and Michelle). Unfortunately, their efforts can’t make up for the dysfunction of the overall system. For the sake of your loved one’s safety and dignity, I strongly urge you to look elsewhere. The Oaks at Conyers, and frankly, the Oaks organization as a whole, should be avoided at all costs.

My family and I found the Oaks at Conyers to be very clean, well-organized with caring, professional, warm & very helpful staff. Our loved one really enjoys living here and is very impressed w/her care. She says that she feels right at home here and that she is treated like a member of a nice family.

I placed my mom in the Memory care area almost a year ago. During the time she’s been here she have met new friends whom I feel she enjoy. As far as the staff everyone has really kind and respectful to my mom. They play games, listen to music, dance and do other activities to stimulate their minds. They have Church on Sundays and mini concerts periodically and celebrate the holidays. They have healthy meals daily. And I must say they have really been a big help to me.
