Shai - while I agree with much of what you stated in review about the facility after having spent the last six weeks here recovering from a motorcycle wreck. I have to say I call bullshit on your statements on therapy. While the food and SOME of the staff Are below the level that I would expect in prison, the therapy group is absolutely amazing! They are the reason I am standing up and walking today That being said the food and some of the staff I would agree are borderline criminal. They couldn’t figure out why I kept losing weight while I was here and wouldn’t listen to the fact that the food was the primary reason - add to that that some of the contracted stuff could give two hoots about the residents and are present only for a paycheck If you have a family member here pay very close attention to their care and feeding. If you were looking to use this facility for long-term care, please consider the pictures I’ve posted below before signing the dotted line. Keep in mind each one of these represents a “complete” meal …. Edit: your reply notes that double portions were requested yes. they RARELY provided that. Look again at the last image - 2 bowls of cereal next to the "eggs". i completed menu requests weekly - again RARELY were those requests honored. if you are proud of your provided meals, maybe YOU should try living and healing on the sub-par provisions. I spoke with staff SEVERAL times while there - one or two meals would change, then it would return to the images posted. Double portions of sub-par food, is still sub-par food no matter how you try to present it.

If I could give negative stars, I would. My father-in-law was in this facility for rehab after a stroke. We were promised hours of physical therapy, occupational therapy, and speech/cognitive therapy. He received less than 30 minutes of all three. The family did more cognitive therapy than anyone working there. He would also sit for hours after asking to go to the bathroom. When we asked for the Ombudsman number, things appeared to change. A lot of people running around with clipboards, and the next thing we knew, my FIL was discharged. The place was very loud, the nurse’s aids were awful and mean, and the place just smelled bad. A family member was always there from before breakfast to when he fell asleep for the night. Originally, when we would arrive, he wouldn't be dressed with shoes and socks on. All of a sudden, he was awake, dressed with shoes and socks. The podiatrist came in to trim my FIL toenails and my SIL noticed something wrong with his one toe. That was the first that anything was done for an infection and it took my SIL to say something. After discharge, he was eventually taken to the ER where the infection was actually in the bone. Chicora staff never did any images to check it. Shout-out to Nurses Sherry and Toni. Shout-out to Eliza (spelling) and Blake, two of the nurse’s aides. We will also be filing a complaint with the Department of Health.

If I could go lower or a negative star I would. This place needs shut down. Smells like a death morgue and no sanitizing smell anywhere In the facility. My Dad has an ileostomy bag and the nurses were aware of my Dads condition and didn't even help empty the bag till it leaked. Then with that you have a health issue with the leaking bag. Bathroom is where it was emptied and rinsed in the sink with the faucet getting residual on it. Wall papered walls that have been painted over that is now peeling and hanging off the walls, unkept conditions in the rooms along with aged equipment that's being used. Makes you feel like your in a Insane asylum, even approaching the building you feel this. Needs to be shut down.

Our 87 yr old Father was a direct hospital admission to the dementia unit. This was a difficult transition due to sudden onset last stage dementia. Sadly he passed only 3 months from admission right before Christmas 2021. We couldn’t have been more satisfied with the care he received from the Quality of Life Center Dementia unit nursing staff, front desk and business office personnel. Praises also to the cleaning staff who kept the facility clean and did not have that hospital odor that is difficult to control. Our Father was always clean, hair combed, clean linen and we were always greeted with respect and kindness. They called us with his care and medical status information and returned calls quickly. You never felt like you were a bother and they encouraged us to call anytime. He was on hospice only one week before he passed, but Three Rivers Hospice stepped in and did the best they could. The nursing staff and other departments went above and beyond, to help his days left, be a positive easy transition. It was late and we had to drive an hour and a half to say goodbye, after he passed, and they waited for us and greeted us with such kindness and caring spirit. We are humbled and grateful for the staff who helped us all through this difficult journey. Guy & Donna Swails

Very poor care and conditions. My wife was sent here for hip surgery rehab and at first therapy session she was hurt! A doctor did not even come to check and see what happened!! 1 shared bathroom for several people and she has to make an appointment to use it!! No phones in rooms. Waited 2 days to get a pitcher of water. I could go on and on. DO NOT GO HERE.
