I was very impressed with our tour guide for being so knowledgeable, informative and she answered every question. The facility was clean and no foul odors. The employees were pleasant and cheerful. Thanks for making me feel welcomed and at ease with placing my love one in your facility in the near future. Thanks a million!

My brother has been neglected in this facility. Not been bathed in a very long time. He smelled and his skin is extremely dry. His feet are swollen and his leg looks like there’s an infection from all of the scratches on them. He’s itched so much he’s clawed himself. The nurse was not honest saying he’s getting lotion twice a day but he obviously hasn’t because of the dryness. She said that he has diabetes and takes Metformin. He said they aren’t giving him any medication. I do know that the nurses haven’t been honest with me before. He was getting a bed bath when we left but he needs a shower regularly and washed thoroughly. Please do better for the helpless people in your care. I hate to see people treated like this especially my own brother.

I am extremely disappointed with the level of care and communication at this facility. It is very difficult to reach anyone by phone — calls often go unanswered, leading to repeated voicemails or having to call back multiple times before someone responds. This lack of accessibility is frustrating and concerning for family members trying to stay informed about their loved one’s care. More importantly, there have been interactions that I find completely unacceptable. A CNA reportedly asked my mother, “Why are you even in here?” That question is inappropriate and unprofessional. A patient’s medical condition is not open for judgment or commentary. Additionally, stating that she was the only CNA available to help at the time raises concerns about staffing levels and response times. I do not know the CNA’s full name, but staff and patients refer to her as “CNA D.” My mother was told that she currently only has one patient assignment because she does not have her TB vaccination completed and that she is working through an outside agency. During conversation, she reportedly stated that she “doesn’t have a boss,” which is deeply concerning. Regardless of employment status — whether permanent staff or agency personnel — all individuals working in a healthcare facility are accountable to leadership and expected to uphold professional standards of conduct, communication, and patient care. I am frequently on the phone with my mother and have observed ongoing delays in assistance. Her roommate, who requires significant help, also appears to experience slow response times. The overall attentiveness and sense of urgency from staff seem lacking. Thankfully, my mother is recovering well from her stroke despite these concerns. However, as someone who works in the medical field, I find these experiences troubling. Patient dignity, respect, professionalism, and timely care should always be prioritized. Greater oversight, accountability, and review of staffing practices may be necessary to ensure appropriate standards of care are consistently met. I sincerely hope improvements are made for the sake of all current and future patients.

Lake City Nursing and Rehab needs a serious overhaul in its operations and name. "Nursing" and "rehab" imply a professional standard aimed at improving people's health. However, my personal experience has been the opposite. If I could rate them negatively, I would. The facility is extremely unprofessional in caring for your loved ones. When you pay for their services, you expect competent care. Unfortunately, they fall very short of this standard. Item theft, lack of proper record-keeping, and negligence towards patients are just some of the issues observed. 💔When you bring items for your loved ones, they often go missing without any proper records. Even worse, patients get bodily injured due to inadequate supervision—they’re supposed to keep patients safe, ensuring they don't wander into others' rooms or engage in conflicts. What they seem to be doing is the bare minimum, just enough to get paid without actually providing the proper care they promise.💸 In essence, it feels like they get paid to merely exist rather than provide any real caregiving. It’s astonishing how far they fall from the professional standards expected. It resembles a negligent cop who parks and sleeps on duty but still gets thanked for his so-called service. 🚨 If you really care about your loved ones, avoid this facility at all costs. Regularly check on anyone placed there if you must use their services, but the best recommendation is to stay away entirely. Save yourself and your loved ones from the heartache, frustration, and disappointment that comes with their subpar care.💔🙏

If I could give this facility negative 5 stars I would. PLEASE IF YOU LOVE YOUR LOVED ONES, DONT SEND THEM HERE. It has been incident after incident with these people. I truly don't believe no one is certified, or has the proper training to care for any of the patients. I know it is very hard caring for the patients, because they all require different care plans. That does not excuse letting my love one be malnourished. The facility is also nasty, disgusting, and dirty. If you do a pop up over the weekend no one is in charge. They have 2 people seeing about 30 patients on a locked ward. Absolutely no one is professional in the whole place. They seem as if its a bother to care for the patients. I called the facility several times checking on my loved one, and I was placed on hold then hung up on. I had to call back five times before someone would talk to me and tell me what I needed to know. I promise I can't think of one nice thing to say about the whole facility.
