Personal Care at Evergreen is a freestanding, licensed Personal Care Home dedicated entirely to caring for senior adults in a home-like setting across three locations in southwestern Pennsylvania and northeastern West Virginia. Operating since 1999, the facility offers 24-hour care including hospice care, short-term respite stays, and resident-focused programs with activities seven days a week.

if i could give this place 0 stars i would. the facility is disgusting to say the least. and they treat the residents horribly and leave them sitting in soiled briefs for hours at a time. i wouldn’t let these people look after my dog!

Fantastic very clean facility with well-trained staff who care deeply about the residents. Staffing ratios are very good. The staff are very friendly and helpful to resident families. There are lots of great activities for residents and their families to engage in each day. We like the new upgrades all throughout the building.
